If the intake worker has encountered the applicant before, the intake worker can provide third party documentation under these conditions:
- To document that the applicant is currently homeless, the intake worker must have physically observed the applicant's living situation.
- To document prior months/episodes of homelessness, the intake worker can document that he/she physically observed the applicant's living situation OR document their encounters with the applicant in another setting. (i.e. soup kitchen).
- The intake worker can only provide documentation for those months when he/she personally encountered the applicant.
If the intake worker's only encounter with the individual or head of household is at the current point in which they are seeking assistance, the intake worker's observation is considered second order of priority.
