An individual or head of household with a qualifying disability can be considered chronically homeless if they are coming directly from an institution where they have resided for fewer than 90 days and they resided in a place not meant for human habitation or an emergency shelter immediately prior to entering the institution. They must also meet one of these criteria:
- They were residing in a shelter or place not meant for human habitation for at least the last 12 months continuously or 12 months cumulatively over a period of at least four (4) occasions in the last three (3) years prior to entering the institution
OR
- The time spent in the institution plus the time spent residing in a place not meant for human habitation or an emergency shelter totals at least 12 months continuously or 12 months cumulatively over a period of at least four (4) occasions in the last three (3) years prior to entering the institution.
