Practice Scenario
For the purposes of this practice scenario, it is June 2016. Jim Smith has been living in a shelter or place not meant for human habitation for the last 12 months (June 2015 - June 2016) and has a diagnosed disability. A PSH program has an opening and is working on securing documentation to show that he has been chronically homeless. He receives medical care at a local clinic. He saw a nurse practitioner on June 2, 2015, July 23, 2015 and August 30, 2015. During that time he was sleeping in his car. Jim stayed at a local shelter from September 12-20, 2015, October 3-5, 2015 and November 20-30, 2015. The shelter uses HMIS. On December 1, 2015 Jim entered a psychiatric hospital. He was discharged on January 3, 2016. During the month of February and March, Jim talked to a client advocate at a local day shelter about how to obtain a state I.D. During this time, Jim was sleeping in a local park. From April 2016 to present day (June 2016), Jim was sleeping on the streets. He had limited contact with service providers and needs to self certify his homelessness for this time period.

- Documentation of Disability
- Documentation of homelessness from the nurse practitioner for the times that they met in June, July and August 2015
- HMIS records of the shelter stays in September , October and November 2015
- Documentation of the stay in the psychiatric hospital (institution) from December 2015 to January 2016
- Letter from client advocate stating that he assisted Jim in February and March 2016
- Self-certification for April, May and June 2016
